Link to a published form

We have a custom form in our Outlook Org Forms Library, a request for help
which has several text fields and a number of check boxes. We want to be able
to have a user open this form from a menu item in an application.

We have tried saving it as an .oft, an .msg file and then linking to it but
when a user opens it, the form is blank. So it appears that we cannot save
these forms offline and still have them work.

Is there any way we could directly link to the form in the forms library to
have the users be able to open this form from their application?
