form is blank when sent to an Outlook folder

I created a from with the "edit compose page" formatting tool selected.  I'm
running in test mode and everything looks great until I send.  When I
retrieve the sent form it's a blank e-mail message.  Whey isn't the form
showing up?

Did you also click Edit Read Page and create a matching read layout? ALso, where is the form published?

I figured out I had to uncheck "seperate read layout" under the forms menu.  
This seemed to link them page or make them one in the same.  Thanks.

Right, you have a choice there: Use a single layout or use two.
