The truth of the matter is that you should have used the forms manager, on
the properties panel of your Outlook file on your Old computer and moved the
forms to the new pst before you moved it to the new computer.  

Now, you need to create a new pst on portable media on your old computer, use
the Forms Manager to copy the forms into the new pst. Now, carry the
removable media to the new computer.  In Outlook, open the new pst, use the
Forms Manager to copy the forms to the pst with the contacts, close the .pst
with just the forms and remove the media from the new computer.  Violin! You
have the forms in Outlook in the new computer.  One point to check, make sure
the new computer has the pst containing the data items and the forms set as
the delivery point for your mail.
