When I try to access a new Appointment. I get this message.

The for you selected could not be displayed.  The form required to view this
message cannot be displayed.  Contact your Administrator.  

I have no Administrator.  It's like the form is missing.  I can access other
forms Contact Form, etc. no problem.  

If anyone could help me with this one -I sure would appreciate it.

Have you tried clearing the forms cache? Tools | Options | Other | Advanced Options | Custom Forms | Manage Forms | Clear Cache
