Publishing Contact Forms

I have totally confused myself....I was successful at adding/publishing a
new contact form, but I'm unable to do it again.  Can anyone tell me exactly
which folder/folders do I publish a contact form to?  The new contact folder
I am trying to customize is called "Volunteers"

You have two choices:

1) Publish the form to the Personal Forms Library, so that it is available to all contacts, regardless of what folder they're in.

2) Publish the form to the Volunteers folder, making it available only to items in that folder.
