What this looks like to me is that you're actually loading a custom contact form, not the standard form. (The layout has been heavily customized; the omission of the Full Name button isn't the only change.) This could be the case if the registry substitution method described at http://www.outlookcode.com/article.aspx?ID=39 had been used to make a custom form the default for contacts. Check for registry entries under HKCU\Software\Microsoft\Office\12.0\Outlook\Custom Forms
Note that the layout that you see for the contact form in design mode really has nothing at all to do with the layout of the standard contact form.

That would seem to eliminate the most straightforward explanation. I don't have any other ideas, unfortunately. I would run Help | Office Diagnostics on that machine.

I just finished running office diagnostic but result is without any error.

I notice default contact form have change after i update outlook with SP1,
without outlook update the full name button still exist. Is it possible that
the update itself have overwrite the existing contact form?

There is nothing in SP1 that I know of that would do that.

I not sure what went wrong here. Anyway the outlook seems to have revert the
contact form back to the original one. I also not sure how or when this has
happened.
