How do I set up a form to create entries/appointments in a published calender
on the web?

I want to use a form to ensure that all new entries/appointments made by any
user conform to certain criteria.

Maybe I'm missing some detail, but I don't see how an Outlook form would have anything to do with a web calendar. Could you clarify the connection, please?
