How can I get Send/Receive to automatically check/download
new mail when I first open Outlook 2003?  Right now I need
to manually click Send/Receive to check for new mail.  I've
made Outlook my default email program.  I've found 'check
for mail when exiting' but now 'send and receive message at
startup'.

To get mail automatically: from within Outlook 2003 open Tools/Options/MailSetup/Send&Receive and check the box "schedule automatically..." and also enter the frequency in minutes you want Outlook to ckeck.
