How do I remove Network Password request in Outlook?

Since I upgraded to Outlook 2003, I have a box which pops up every couple of
minutes, when Outlook is open.  At the top "Enter Network Password", in the
box "Please type your used name and password.
      Server   -------
      User Name ---------
      Password --------
etc."  
How do I get rid of this request permanently?  (If I click Cancel then it
goes away for a couple of minutes but comes back.)
