Lookout has disappeared from my Outlook

My Outlook had crashed and gave me some error stating that Lookout was
affected.  Unfortunately, I had paid much attention to the error message at
that time.  The next time I started Outlook, it was no longer showing up
regardless of whether or not the Lookout Toolbar was selected to be visible.  
I have since uninstalled Lookout, restarted my computer, and resinstalled
Lookout.  Outlook never recognizes that Lookout has been installed and it
does not list the Lookout toolbar has an option.  Any suggestions?

Outlook 2002 or 2003? See if it's in the disabled items list. Help, About
Microsoft Outlook (About Microsoft Office Outlook for Outlook 2003),
Disabled Items button. If it's there select it and click the Enable button.
Exit and restart Outlook.
