I have this installed but can not tick it in COM Add-Ins and therefore no
toolbars show.  I have reinstalled the product, no change.
ANy suggestions please?

See if it was disabled. Select Help, About Microsoft Outlook, Disabled Items
button. If listed there highlight it and click Enable. Exit and restart
Outlook.

I installed it at Outlook 2003 and 2007. I have got the same problem but
there are no inactiv or disabled add-ins.

Any ideas?

You said you can't check it in the COM add-ins dialog, does it show up there
at all? What happens if you check it if it is there? Does it show any
errors?

There is nothing in Outlook whats about the add-in. The installation ends
with no error or something but when i start outlook, there is nothing at all
what looks like an sms add-in.  Thats my problem.... :/

I have no idea then. You might contact support for that addin, that's about
all I can think of.
