Is there a way to add a disclaimer to all users signature for outgoing
email?

A disclaimer of that kind is best added by the outgoing server and not by
the individual clients.

can this be done with exchange 2003 and NOT with an additional addon or 3rd
party product?

That's a good question.  I don't know what we use, but all our outbound
messages contain a disclaimer - even though tests in court show the're
rarely work the electrons they consume and do not protect the sending party
at all.
