I want to rotate a image imported from an Adobe Acrobat file. All rotate
options are grayed out. I do not have full Acrobat so I can not rotate and
save the source.

Try copy > paste special as a png file. See if that rotates

What version of PowerPoint do you have? You can't rotate images in earlier
versions.
You can copy/paste the image from Acrobat into an image editing program, rotate
it there, then save it and use Insert, Picture, From File to bring it into PPT,
though.

Using pdf2picture you can convert any graphics drawing or form saved in PDF
file
to Windows native graphics format Windows MetaFile (WMF).
The WMF file can be imported (Insert Picture...) and edited using MS drawing
tools.
pdf2picture is also able to rotate the PDF-file.
