Right click on the movie, select Edit Movie Object.
Put a checkmark to "Rewind movie when done playing". Click ok.
Works for me here. See if it helps.

I believe all the effects are set to "After Previous"?
Seems like you can't add the fade out effect immediately once the movie ends.
So how about this:
Double click on the fade out effect.
Click on Timing tab.
Set the delay to 5 seconds. (Or more)
Click Ok.

Try using faded zoom (medium) as the exit
