You can't do this with current versions of PowerPoint.

While the current versions of PPT don't allow you to create custom layouts,
PPT 2007 (which is beta) does. If you are using PPT 2007, let us know so
that we can help you better. If you are using one of the older versions, you
have to fake it by adding slides that are laid out the way you want them
with text boxes, then using copy and paste to add new slides.
