David,
File - Page Set-up - select A4 - portrait or landscape - OK

Is there a way to have the slide notes in A4 but keep the slide show in "On
Screen" size? The reason I ask, is that I have a client who when viewing the
show in A4 has black horizontal bars showing on the top and bottom of the
slide. I don't have this issue, but he does. Any ideas why?

Go at it the other way around ... set up the presentation as On Screen Show.
When you (the client, anybody) print, PPT will fill the page reasonably well,
whether it's letter or A4, especially if you select the Scale to page option in
the print dialog box.
