I want to print one handout per page, vertically aligned at the top of the
page.  The default setting is centre aligned vertically.  If have gone into
the handouts master but cannot select the “placeholder” for the slide.  How
do I achieve my aim?

I am using 2003

You can't change the built in PPT handout layouts, unfortunately.

But since you only need one handout per page, you can use Notes pages instead.
You can modify the notes master in pretty much any way you like.
