Does anyone know how I can create a cross reference or an index reference in
PPT 2003?  I would like to automate my table of contents based on these
references as well as create an index at the end of my presentation giving
people the page numbers of key subjects.

Thanks,

For table of contents, have a look here:

Create a table of contents (TOC) slide with links

I don't know of any software that helps with indexing.
