I've created a template for a client in PP 2003.  The template formats the
slides, title, handouts and notes masters.  When I apply the template to a
slideshow, the slides and title slide are perfect.  But the formatting on the
handouts and notes masters do not seem to get applied.  Am I doing something
wrong or is it not possible to impact these masters through a template?

It depends on the way you apply the template to a slideshow. If you use
Format - Design, only the slide and title master are applied. Open a new
presentation based on this template instead and use Insert - Slides from
file to import the existing slides. That will apply the handout and notes
master, too.

In addition to what Ute said, save the original file as a PPT/PPTX file, not
a template. Template files can only store slide and title masters.
