Template Problems

When applying a custom template, a random text box shows up.  We can't
seem to figure out why this is happening.  It's not on the slide
master or on the original ppt we created the template from.

Click View > Master > Slide Master.
Click Format > Master Layout.
Is there any placeholders that are uncheck? If so, put a checkmark on it.
Close Master View.
