Template of Spiral notebook that turns pages

I'm using PowerPoint 2003 and need my presentation to appear to be a spiral
notebook.  My bullets would actually be text on the lines as if manually
writing on a spiral notepad.  I've seen presentations like this before but I
have no clue how to create one.  Thanks

You will need 3rd party software such as Crystal Graphics' PowerPlugs add-in:

and Ovation:
