Creating a cover for a perfect bound book

Hi all. I am working on a small book and need advice on creating the cover.

When finished, the book will measure 6"x9", and will be no more than 108
pages. It will be printed perfect bound. How in the heck do I design the
cover? Obviously it will need to be two 6"x9" pages plus the spine, but how
do I calculate the width of the spine?
