Why do I get an error when I submit my Web form?

I inserted a form through the insert object function in Publisher.  I
published the page to the Web.  When I went to the Web site, completed the
form and clicked "Submit" I received an error message indicating the page
could not be found.  What am I doing wrong?

I posted a reply to a slightly newer form question from 12/15.  please take
a look and let me know if it helps.  Cheers.
