adding a form?

i want to add a form that is sent through email back to me into my website i
would like to have a couple check boxes capture name address email and a
short question - message text. then submit through a button that would copy
contents into the users email and send to me.

Publisher has ready made forms in the templates.
Or you can use the form controls under the Insert menu.
