Using Submit Button To Send Email

When I try to setup a web form with a submit button to send an email, it
fails with the following error.  My host does use Frontpage 2000 server
extensions.  Any suggestions?

Cannot run the FrontPage Server Extensions on this page:
"http://www.odysseyearlyschool.com/index_files/Page1472.htm"

Go to http://www.odysseyearlyschool.com and click on Suggestion Box to
recreate the error message.

As per http://www.publishermvps.com/Default.aspx?tabid=30 please verify you
upload the web via http and NOT ftp.

If http is being using then have the host reinstall the server extensions.
