HOW DO I ADD A PRINTABLE ORDER FORM TO A SITE

I use MS OFFICE XP PRO edition.  I have a long product list in a table, which
is printed out by customers and faxed back to me.  Is there a way to turn
this table into a shopping cart type of deal, where the person can add which
items they want to a "check out" page and print it out.  The purpose would be
to condense the table to only those items that they need, so that when they
print it out, its 1-2 pages and not the current 5-6

Under the insert menu there are form controls, you can build a form. Pub 2003
includes a few form templates.
