Don, thanks for the reply. I have the 'Submit' button positioned on the form
and have identified my e-mail address in the submit area, but when attempting
to send the e-mail, the program cannot find the server.
I can replace it with 'http://www........ and it goes through just fine so I
think I need to identify my ISP along with the e-mail address but do not know
how to do so.
What Don speaks of is different then building a form and having that form
post to an email address. You weren't clear in your question, not phrased
well. The topic of forms and how to use them correctly is covered in the
forum constantly, for example you can refer to my response to the thread "how
do I create a form" posted earlier today and to another thread just a couple
posts before that. Also forms are well covered on the FAQ -

Thank you David, I visited the FAQ link that you suggested. I am a total
novice at web design and while you probably answered my question in the FAQs
I missed it. I have not published my web page to my ISP site and am testing
the design with Publisher Preview. Perhaps the FPSE you allude to in some
FAQs is missing or perhaps I have to load it on the server site for it to
work. Jack.
