I've changed the name of my default home page to index.htm by adding
"/index.htm" at the end of my homepage url. In fact, if I go to the regular
url (it gives me an acceess denied kind of message) and physically add the
"/index.htm" I see the site with no problems. The ISP has set up my site so
that index.htm is the default page. What do I need to do so that the
index.htm page is what people see when they log in?

It sounds like you are using Pub 2002. If so, then read the following
article, and especially the third paragraph.

As DavidF pointed out, version 2002 does not default to naming the web
"index.htm" so you have to explicitly do that when you save/publish.  (PLEASE
NOTE: Always State The Version Of Publisher You Are Asking About In Your
Post). A web server is always setup to default to index.htm or index.html,
that means that as long as you uploaded a file with that name then using the
URL "www.mydomain.com" , IS EXACTLY the same as using the URL
"www.mydomain.com/index.htm"
