Form format and submit issue

I've created a website using Publisher 2003 and uploaded it using the HTTP
method. I'm still having trouble, however, with the Request form in the
webpage. The form is located at www.woofsandhoofs.be/index_Request.htm .

I've read through the FAQ page
http://www.publishermvps.com/Default.aspx?tabid=30 , but can't seem to get
any satisfaction out of my site.

Any thoughts on where I am going wrong would be greatly appreciated !

First red flag I see is the use of both upper case and lower case in your
file name. I cover that no no in my article

First order of business... make all names lower case and make sure all links
and that submit button are all lower case. Write the following on a post it
and attach to your monitor - "web design = all lowercase all the time"

Then delete the site files from the server and save and publish fresh. And
test it.

If that doesn't correct it the next red flag is that babel fish code insert.
Strong possibility that code will interfere with the form code. Remove that
code fragment and repeat the clean publish and test.

If you still have the issue then tell your web host to reinstall the FP
extensions. If you make an ftp connection to your FP extension enabled site
you risk corrupting the installation which requires a reinstall. Then make
another post it and affix it to your monitor opposite the previous one, it
should say - "publisher forms = http publishing all the time"

When they confirm that is done then repeat the clean publish and test.
