No Form Control Toolbar

The Form Control toolbar is not listed among the Tools>Customize list? How do
I get it loaded? Any ideas please.

There is a form control icon/button on the Web Tools toolbar.

Thanks - I now realise that I needed to convert doc to web first!
All OK now.
