How to read text from a form, into report?

I have created a collection form, including a sign up sheet, with check
poxes.  I want to do two things.
ONE I copied form entries including the check box and text box, now EACH
copied box has the same ID as the sample I copied.  How do I change this?
TWO when I insert a check box, I want the report to return the text in the
appropriate text box selected.  How do I do that?
