Is it possible to assign a text box a specific text or numerical field. For
example, if I want a date of birth from my customer, can I assign the
customer response field to respond in mm/dd/yyyy?

Not sure that you can do that with the form controls in Publisher, but it
might be possible using a different form program.  Reference: Publisher web
