Add name request to Publisher 2003 reply form template

I am trying use a reply form template in the desgn gallery on my webpage. I
have deleted the fields that I do not want on my webpage but I want to add a
place to put a Name request on the webpage. I added a text box but when I use
Webpage review it does not appear the same as the other fields ( it is not
colored yellow and the others are).

Perhaps Insert > Form Control >Text Box.
