Web form not working

I have built web forms using Publisher 2003 on several websites successfully
- until now.  

I read about DavidF's references on "Publisher web publication forms 101",
"How to publish a Publisher web in HTTP", and "How to trouble-shoot Publisher
web upload issues" recently and tried to solve the problem by deleting the
"troubled website" from the hosting account and re-built a new one from
scratch, but it still didn't work.  

Please help!

The entire page is a form! Just make a form on a page.
Try it with Text results instead of HTML results.

In addition to Rob's comments...

I found several forms, and am not sure which one Rob was talking about, and
am not sure why they don't work. As you have built sites with forms before,
ask yourself what you did different this time. Perhaps the FPSE have become
corrupt on the server. Either uninstall and reinstall them yourself, or have
your webhost do it for you. Delete all your old HTML files off your server
space, and upload new files via HTTP protocol...

I will continue to think about it, but I didn't spot anything
obvious...other than I bet you want to use "Portrait" vs. "Protrait" on the
quote page.

Rob, can you please show me how to get text results from web form, thank you.

Oops! thank you David for "shoulder-tapped' me about the error, corrected it
now.  

While trying problem solve myself i did think about what i've done
differently was creating two forms in one folder, so rectified it by delete
the quote form from index files and saved it in order_online files.  And when
that didn't work I then removed the entired web files from hosting account
then republished, and that didn't work either.

So here i am talking to you experts. Please help.

Under Help in Publisher type "forms", and there is a pretty good
troubleshooting section, and other information. Included was:
When adding form controls to a Web page, do not overlap them with other
controls or objects and do not use these special effects on them:

 a.. Rotation
 b.. Fill color
 c.. Fill effects (tints, shades, patterns, and gradients)
 d.. Shadowing
 e.. Borders or BorderArt
 f.. Hyperlinks
Aren't your forms color filled?

Hi David,

it didn't work :(
i even went back to hosting account and remove the old folder and reloaded
the modfied one and it still didn't work.

what should i do????

Fiona,

Did you try removing the lines seperating the form controls, from your
newest version of the page at
http://www.zapwall.com/order_online_files/quote.htm ?

Did you go to Help and type "forms" and read through the topics, especially
the troubleshooting section? Did you double check that you had the Submit
form control setup correctly? Did you check the Form Properties to see if
the email address and such was correct?

You aren't using a Master Page are you?

Have you contacted your host to ask them why your form may not be working?
Just give them a link to your quote.htm page, and tell them it is supposed
to be supported with the FPSE...but doesn't. Perhaps have them reinstall the
FPSE?

Totally guessing at this point....

You should try creating a new form on a blank page with no other junk on it.
