Forms don't show option or check boxes in web page preview

I have created a reply form using the form control from
the objects menu bar.  The option and check boxes are not
displayed in the correct place on the web page preview,
and that is the only time I can see where the preset
option and check bars are displayed.  How do I MOVE the
preset option and checkboxes to align with the INSERTED
ones.

I am not using one of the three predesigned system forms -
should I be?

You failed to state your version but I'll assume you are using 2002 and
