Web Form Submit Button

I use Publisher to create my web site.  I have a customer reply form that
concludes with a Submit button and a Reset button.  I have the Submit button
set to email me the customer responses.  However, when I test the form, I get
a 404 error: Page not found.  I am at a loss.  I have read every
troubleshooting file I can find and attempted to correct accordingly, but the
results are the same.

Please help!
