I created a website with 5 pages and used the publish to web function on
Publisher and this created the file index.htm.  Only one page was uploaded
sucessfully and I did not get any of the content for the other pages.  I did
see the names of the other pages at the bottom.  How do I upload the other
pages on the website?

Publisher 2003 produces an index.htm file plus an index_files folder that
contain your other pages and supporting graphics. Upload that folder and you
should be good to go.
