publisher website form wont email back to me!

i created a website with publisher and added a form to it. but for the life
of me, I cant get it to email me the information back.  I am not very
knowledgeable about forms in publisher. all this html codes and stuff are
driving me nuts.  can anyone help me maybe with an existing code for a
contact us form and how to set it up.?  my site is www.aztekawireless.com.  I
havent launched the site yet I just want to know how to get the form to email
me.

Chris,

Did you upload your gdform.asp to the same directory (index_files,
usually) as the page? It doesn't appear so.

FWIW, allowing access to a script in a directory that is easily
available to anyone is usually considered a huge security risk. Hackers
could divert your Success page to whatever they may want, such as
www.view_porn_and_buy_drugs.com.
