Link submit button to Thank You page

I have my submit button set up to email me the information once the submit
button is selected.  When I hit submit I get the 404 error code message.  I
was told that in order for it to complete the process I need to link the
sumbit button to a thank you page.  I've created the page but can't figure
out how to link it to the submit button request.  Any ideas?

Is the HTTP uploading protocol link down?
There is  line that states Publisher-Uploading and that's all that shows up
on that page.
