I have an application page on the website I manage at work,
auglaizeacres.com, and I want to know if I can make the info come back to me
in the form of the original page. All of the info currently comes back
looking like a shopping list, one line of info at a time.
Field1, Name-First, answer
Field2,Name-Last, answer
Field3,Name-Middle, answer
etc...
Can I do this?

Sorry, but what you see is what you get. Alternatively, look to your web
host for a different form program that is more customizable. Most provide
something, and you can use that instead.

The best way to do this is to use a pdf file that the fields can be
filled in.
