Word Form in Publisher??

A client wants to use the Word form that he has on the site that we created
in Publisher.  I've checked around and can't find anywhere that tells me if
this can be done.  I tried saving the Word form as HTML and inserting a code
fragment into Publisher, but it didn't work at all.  I know how to create a
form in Publisher itself, but he wants to use the original Word form if at
all possible.  Any help would be greatly appreciated!!

Depends on what the ultimate goal of the form is...is it to be filled out by
the user offline and emailed or are you trying to create a online
interactive form on the site?
The former would be best done by creating a fillable form in a PDF creator,
the later...no...you'd have to recreate the elements from the Word form in a
Publisher's form

It would be to create an online form - sorry I didn't include that in my
original post.  We will just recreate it!  Thanks!
