Publisher 2007 - SUBMIT button won't work

I have a website up and running however when I click on the SUBMIT button on
my form page, the webpage cannot be found.  I have clicked on "allow front
page extensions" and have entered my email address in the form properties.  
Any suggestions?

Your web server must have FrontPage Extensions installed and working
You must publish your web via http:// NOT ftp
Your web host must allow FrontPage form handler to work
and...99% of the time your email address must be within your domain... like
http://www.fredsworld.com  email would be you@fredsworld.com

Which operating system are you using, and which version of which browser?

It is written for Pub 2007, but if you are using IE7 instead of IE6, then
the directions for using Vista also would apply to Pub 2003. FTP and HTTP
uploading has changed with the introduction of IE7 and Vista.
