Attach a form to a web page

I am creating a website.  I want to add a form that someone can download and
either fill in and print out or just print out to fill in and fax to me.  Can
anyone give me step by step directions.  Thanks!

If your ISP provides for form submission, consider using the publisher
version of forms.  Much easier than printing and faxing,  The completed form
shows up in your inbox of your email.
