Publisher 2000 and Web Reply Forms

I have tried to create a web page and insert one of the
design gallery's Web Reply Form.  The form inserts just
fine but when I do a web page preview it is all messed
up. If I go back to design mode and remove all the
groupings that were on when inserting the form and then
review it looks ok.  Anytime I group a from control with
the associated text box and preview the page the control
is gone.  Remove the group and preview and both are
their.  Is their something I am doing wrong.  Like I said
even the design gallery's forms have the same problem.

You sure it's version 2000?

If so I'd suggest sending me the pub file for a review.

I am truly sorry.  I am working with Publisher 2002 with
SP3.
