Setting up repeating binder spine size in Pub2002

In Publisher 2000, how do I set up different binder sizes as well as
selecting multiple binder sizes per 8.5x11" page without having to click and
drag?

The existing template displays 2", 2 1/2" and 3" spine sizes but the 1 1/2"
and other sizes that use to appear are gone.

Avery has a template for Word, you can adapt it for Publisher.
