Book printing 13x19 with signatures

I want to print a book 9.5x13 on SuperB paper with signatures (4-8 page
groupings).  How can I do this?  I thought I could use the save as PDF but
that doesn't work.  Tried using PrimoPDF but can't figure out how to get the
paper size in.  Also, publisher will only allow booklet or single sheet - not
book fold.  

I thought I could send the file to Kinkos to get it printed but they can't
do paper larger than tabloid (11x17)!?  Unless they use a roll but I wanted
to use parchment paper.  So then I thought I'd print it myself on my HP
DeskJet 1220C, which takes the paper size if I could just get some kind of
software to cooperate.

Any help would be appreciated.

Help a bit. Publisher version? Is your final book going to be 9.5x13 or
9.5x6.5? Have tried booklet with custom size?
Would you explain the difference between "book fold" and "booklet?"

Publisher 2007.  I want the book to be 9.5x13, so I want to fold the paper
once.  I think that is set up but I also want to print the signatures so that
I can bind the book - which requires grouping the pages so you don't get one
huge pack.  I couldn't seem to do this in Publisher but I thought PDF might
be able to.  The save to PDF addon is not working - I've uninstalled the
trail and installed the addon again but no luck.  PrimoPDF doesn't give
SuperB as an option and I'm confused by explanations I've found as to how to
set up the custom - I editted the ppd file and rebooted but it still didn't
show up.  Thanks for helping, Lori

Book fold is the same as booklet. Setup up your printer for your size paper,
then select booklet. Publisher should default to your paper size. My printer has
a Letter.Extra, Publisher always defaults to this size if I select it first.
