Try this, Alt+V, click toolbars, customize, Toolbars tab, click reset. In the same
dialog, Options tab, Reset menu and toolbar usage data.

Yep, tried that but no joy.
I do wonder whether I have somehow managed to inadvertently move the menu
bar off screen or something. If so, is there any way of getting it back to
its default state??

Can you see the title bar of the window?
(The bar across the top above the toolbar with the Control Box [Publisher
icon], the name of the document, and the maximimize, minimize, and close
buttons on)
