Footnotes that appear on a page before the "insert"

I have a 30 page document where footnotes seem to get corrupted. In addition
to appearing at the bottom of an earlier page, they also skip some numbering.
This is as though there are hidden inserts. When I do a find, there do not
seem to be any extra inserts.  

Any suggestions?

Is track changes enabled? If so, you have to accept all changes and
turn off the tracking in order to make sure that numbering is
correctly displayed.
