Converting Works form to Word Doc or Access form?

Wonder if someone could tell me how to convert a form I
originally setup in Works 6 Suite database, to either a
Word Document form, or to an Access form?

Also, I don't know how to receive someone's response here.
I am giving my e-mail address if you can be kind enough to
use and send reply to me directly.  
Or inform me how to come back here and find my responses.

Hi Lillian,

There really isn't a conversion process for the
data forms in the Works .WDB database files.
The Conversions Plus package from http://dataviz.com
can convert the WDB files but not the form layouts.

You may want to check with the folks in the
MS Works newsgroup for other possibilities
 news://msnews.microsoft.com/microsoft.public.works.win
