You can download a very elementary Word template from
http://jay-freedman.info/cdliner.zip.
If you have a lot of cases to make or you want to include graphics,
though, Word is the wrong tool. You'd be best served by looking on
Google for a dedicated program -- use "free cd case creator" (without
the quotes) as a search term.
